RSU 29 Student Transportation Registration Form

Dear RSU 29 Parents and Guardians,

Please complete the RSU 29 Student Transportation Registration Form if your child requires busing for the upcoming school year or if there have been any changes to your child's transportation schedule, pick-up/drop-off locations, or address from last year. If your child's transportation needs remain exactly the same as last year, no update is required unless requested by the department.

Form Details Requested:

• Student Full Name, Grade Level, and School

• Primary Physical Address

• Morning Pick-Up and Afternoon Drop-Off Locations (including any change requests)

• Parent/Guardian Emergency Contact Information

• Special Transportation Accommodations (if applicable)
